How do you determine Ph with no soil test? Is liming never done?

Another question re. removal rate. First, I assume it's Dap/K? or just P/K? Let's say you fertilize for 200 bu (just a number) corn. Year is good & corn makes 250+. How is that accounted for? One would assume the extra nutrients were taken from existing soil nutrients. Will they be replaced, or just move on & forget it?

Do you vari-rate the fertilizer for known soil types? Some parts of the field may never produce 200 bu no matter what.
