In Iowa its set by state regulations and has to be inspected by the county sanitarian. I bought this house in 2009 and just missed getting by a new law that required the septic system to be inspected and approved when a house changed ownership. The county sanitarian gave me a couple years to get a new one since there was no record of the original having ever been inspected. Cost about $7000 with a new 1050 gallon tank and new drain field, three laterals about 75 feet long after a percolation test in the front yard. This drain field is with the tents, not slotted tile in gravel. The tents take less linear trench but their trench is wider. So far it works fine. The house is considered to have three bedrooms. The old tank was buried deep with no chance of cleaning it and it was only about 200 gallons volume. Probably been there since the house was delivered in 1978.
