Columbia Basin, Ephrata, WA | Here is my attempt at answers, based on helping an aunt and uncle briefly when they built their icf house:
1. They're relatively do it yourself friendly, but it's still best to have a small army available for pouring concrete and interior framing.
2. Garage can be poured the same way, one piece with the house.
3. Yes. Trusses and the main floor (assuming you'll have a slab in the basement) are attached by bolts you put in the concrete.
4. Pipes and wires can be routed through concrete if needed, but for most of them you just kind of hollow out a little raceway in the interior styrofoam.
5. Can't really help with info, my uncle is pretty much my only source of info on the subject. He studied icf homes extensively, but that was nearly 20 years ago.
