We were told by our insurance agent it can be a gamble to pay out of pocket.  The first claim probably wont raise you insurance.  If you pay out of pocket for it and he has reoccurring pain or problems from it, it will be out of your pocket all time.  I guess it is up to you and how you feel your employee would handle it.
