Thanks for the help . . . I was hoping that a pdf form could be saved/converted to something like word doc or rtf or something. I did find a program that allows a PDF to be filled out if the PDF isn't active or fillable - but it's not perfict, so the filled out form looked like crap when done, in this case . . . .

I ended up filling the forms out with pen and running copies for my paper file . . . .
