what antifreeze was installed and at what concentration? It should be no higher than 50% and closer to 30%.
When flushing use Muriatic acid diluted to around 6 to 1. You may need to test your water for nitrates or other organic compounds http://treatment.hohwatertechnology.com/category/chemical-treatment...
if your running a high water temp it may be breaking down the glycol.
