Elderly drivers, especially diabetics, may be suffering from peripheral neuropathy and have little or no feeling in their feet; therefore, they may be confused as to which pedal their foot is on.
BTW, my sister, a nervous Nellie, always drove with two feet; and the left foot rode the brake pedal enough that her stop lights would be on at times.
