Hub pilot wheels have a nut that holds both wheels on. Both sides of the truck are right handed threads. Stud pilots have a nut that holds outer wheel onto the inner stud that holds the inner wheel on. These have right and left handed threads. Left handed threads have an L on the end of the inner stud.

My 95 Freightliner FLD120 has hub pilot wheels and all nuts are right hand thread. Not sure on other brands.
