go for the mri... if it is going down a leg some that could be siatic nerve involvement..... do not use heat more than 15 minutes out of an hour... the body will actually adjust to it and cause swelling..... i had some severe back pain and actually found ice worked better..... freeze in small paper cups and then peel the cup down and rub the ice across his back for a few minutes at the sight of the pain... the cold numbs the nerves some and helps break the cycle of pain... the cold gel packs also work but again no longer than 15 minutes out of an hour... I think it is time for an mri before possible nerve damage could become permanent.....
