Could be many things, but like mccrops said, check for stuff rolling around inside the feed rolls.  In my case, I fought the same thing for a couple years.  Finally pulled it all apart in the winter and found about a cup full of filings inside the upper front roll, built up over time from bearing and bushing wear I guess. Wasn't all ferrous, but enough of it was to cause the issue.  Worked fine since.
