I was told there was a bad batch of O-rings for the cylinder sleeves, IIRC. They shrink over time, and let coolant and oil mix. Unfortunately, they did not track the batch to certain serial numbers, etc. They have an additive which prevents the issue, supposedly almost perfect success rate if the additive is added.
