Seen it before.
Wind blew the glass, as it's very lightweight. You should have air chutes fastened to the underside of the roof plywood. These allow air flow from soffit to get to the roof vents.
They are 4 ft long (usually a styrofoam product) so the top end will protrude past the level of the loose attic insulation, and you use fiberglass batting to seal the bottom end before blowing the loose fill in.
