Texas | A couple of years ago, one of my mom's cats had a similar ear issue. Mom took him to the vet and was told that a blood vessel in his ear had broken most likely from shaking his head too much, and he probably had ear mites.
He was given ear mite medicine and the vet told my mother that the blood would go down on its own with time; however, the cat would then have one crinkly ear. The vet diagnosed Mom's cat correctly, and yes, the cat now has one crinkled ear.
If your cat has the same issue it may be a more severe case, as your cat's ear appears larger than I recall my mom's cat's ear being. I am just sharing what appears to be a similar issue; I do not know if it is the same thing your cat has or not.
The $500 quote that the vet gave you sounds outrageous to me. If another vet is an option, I would probably get an additional opinion.
