The rate billed is all over the board, sometimes actually less than medicare rate but averages about 40% over.
I know it's available someplace, you can find the medicare payment rates and a fair price is a bit over that. Sorry I don't have it handy, what trips a lot of people up is the codes and such for procedures but if you have the bill, you should be able to tell what is what.
