I'm a paramedic, work for a fire department that does transport EMS. Have several coworkers that used to fly. Most didn't like it-most flights are interfacility transfers.

One guy I know well told me he worked 24/72, meaning he worked 24 hours on and 72 hours off. All the flight crews at this service did that, meaning they had to have four, three person flight crews. They also had two full time mechanics. This service was through a hospital so dispatch was handled through another part of the hospital as a side duty; otherwise there would also be a need for probably four or five dispatchers.

What I found interesting was that at this service, they had no "relief" crew members. If you were sick or otherwise wanted a day off, you had to arrange a trade with one of the other three people qualified for your position.
