I fight this same thing with my '88 Ford constantly. Causes have been leaky return cap o-rings, bad brass valve on fuel filter housing and bad lift pump. Winter time cold temps exacerbate the problem as it never has a problem in the summer, also for some reason using the block heater also improves its behavior. On the 6.9/7.3 forum of another website, it was recommended to replace the mechanical lift pump with an electric Holley "red" pump, I haven't done this myself, but can see the benefit to it.
