Been there, still working on it.
Have a conference with current teacher and include her in making a plan.
Get the child tested for all of above mentioned.
Find a trained tutor. (we found special ed teachers to be best)
Read every night. A book series is good for this. Alternate with child.
Read everything in daily life together. Signs, school posters, ads, etc
Start now & don't give up.
