Thanks for your replies as well as everyone else's too.

By all indications the broken head bolt has been broken for some time as the break was not new at any place. I suppose it could have been broken for quite some time and somehow the head of the bolt finally popped out.

The gasket and all bolts will be replaced and special attention will be paid to the instructions.
