I have the exact same thing in one now as well. Never had one like it before, a month old, basically healthy, tried IV antibiotics, got somewhat better with dex injections for a couple days and now is relapsing. Really nothing left to try, he's got another 24 hours to show signs of improvement as far as I'm concerned.
