Agree on documentation. Assuming someone knows what not to do is taking a big risk. If thereH are concerns about people performing unsafe acts in the work place a simple set of safe operating procedures would be a starting point. Have employees read them and sign off on them as ''read and understood'. At least you can have a clear conscience know you have made an effort to insure employees safety.
