| Yep, a fault somewhere and some minor stress.
Could be not allowing for temperature dilatation in the contact between the glass pane and the frame or hinges. A slight temperature change could have exploded it eventually.
Glass is very tricky. I once visited a glass factory in Albi, France. They had a big press weight falling on each bottle to test their solidity. I couldn't even believe that a bottle could survive the test, but the same bottle can go under the press many times without breaking. But take one that passed the test, put a few grains of sand in it, shake the bottle, put it back under the press and it will explode. The shaked grains of sand were sufficient to break the internal resistance of the egg-like glass structure, even though the microscopic chips were not visible. By the way, it was the first co-op in France, founded in 1896 under the auspices of the first French socialist president, Léon Blum.
