I had that happen once last year, but haven't seen it since.  My firmwares are a 4.something this year.  The speed exceeded can happen if you drop lots of sattelites fast and regain them.  I throws the guidance into thinking it's over there and then back to here, so fast that it freaks.  I see this most times when I drive into a shed with the monitor on.
