I could tell you what I have seen done by others in 35 years of practicing medicine, but that has no bearing on your unique situation.
I can only tell you what I will do if/when I find I have an end stage, fatal disease.
I will NOT prolong my life a few miserable months by making myself deathly sick(er) with radiation and chemo.
I will spend as much time with those who care about me, as I can.
I will get everything signed over to her name, right now, snicker snack. (most already is) (get a lawyer to tie up any loose ends)
We will decide what to do about retiring debt so that she is not left without adequate working cash.
I will immediately sell the tools, and guns, and cameras, and ham radio gear, and telescopes, and airplanes, and boats - because I (and only I) know what they are worth and how to get the best price in a rapid sale situation.
Ebay and Craigslist are your friend in this situation.
The farming tools go to my son so I will just sign them over right then.
The land is already in her name.
And if I have the strength, I will take a last trip. It does not have to be anything special - could be just to see the Grand Canyon, or sit on a bench at Disney Land and watch the young families laughing and making memories, or Bass fishing, or whatever.
I will insist that my doc give me adequate pain medication so that I am not in agony.
I will stretch the use of the drugs so that I have an adequate last supply for when I need it.
I will insist on cremation, a simple Irish wake, and scatter my ashes to the wind.
