I think it is too late.
The county and state pick up the nursing home bill after your assets are used up and the person goes on to Medicaid and they don't take kindly to folks impoverishing someone to save their assets. They will not take the surviving spouses home, but the farm may be fair game. They can also put a lien on the property if it is not sold.
