Also check the pivot assembly under the cab. I have had several with the hardware loosening up that fastens the pivot assembly to the cab support. They will shift up or down a gear intermittently while driving. There are bushings in that pivot as well that can become worn. Ball joints on the linkage become worn as well. Good luck.
