Just updating the post with the resolution.
After lots of troubleshooting, where it looked like it was fixed, and was not, it was the bottom switch that was causing the issue. The top switch had been replaced multiple times, but the bottom switch was acting up intermittently and causing the top switch (appearance of) failure. Thanks everyone for help/feedback.
