Latching onto that tidbit, this is usually what happens. Multiple small errors stack up that lead to disaster. It's very rarely one simple cause. Anything from maintenance, ground crew, pilots and even occupants can lead to individual mistakes that won't take down a plane, but when combined together can lead to these accidents. |
