When I was in school we had a situation where we had an aunt and nephew in our grade and there was no second wife involved.   The aunt actually had two nieces that were even older than her.  

 Grandma had gotten married young, had a baby girl who also got married young and had kids, and then Grandma had a very late in life pregnancy.
