I got it from fall off a train car ladder and landing on my feet then my back. It was miserable. I went to the good feet store and they sold me a $400 pair of inserts. Those inserts were so painful to wear I couldn't bear it. (brother went in same store and thy have cheaper inserts that aren't nearly as expensive that he said helped him). I just put up with it for a month and it finally got better.
