You should be able to drive steel pipe from a hole over by your hydrant to a hole that you drill through your concrete. Of course the pipe will have to be capped so it doesn't fill with dirt. You can keep adding pipe with couplers until you reach the hole then remove caps and run your poly hose through the pipe. Aim carefully when starting.
