South Central MN | I had a rather large cone shaped chunk guard that I welded up for one particular bin, was at least 2' around and a bit taller since it was a steep cone shaped thing, widest gaps spaced enough that anything getting past it could fit through the sump. (Older bin so just the typical sweep auger support bracket in the middle.) I haven't used the bin for several years now though (needs a floor rebuilt or aeration floor put in) and the other one doesn't get crusty on top like the first one did.
For the one bin I do use frequently I always check the top before starting to unload, if it looks questionable I can at least break up the stuff on top first. Otherwise I stay out of the bin until it gets down to the sump where the grain won't flow anymore, then it doesn't take much to get the first door panel out. After that it goes quick.
Main goal of the guard on the one bin was to deflect chunks to the side of the flow, and allow a large area for grain to fall through. It also had a secondary sump near the door if needed but had to be careful it didn't start lifting the wall off the pad.
