microbiota might be in play.

Article suggests this: "A low intestinal absorption, due to the P glycoprotein (Pgp) efflux, a presystemic degradation by microbiota..." https://pmc.ncbi.nlm.nih.gov/articles/PMC8665891/

>presystemic degradation by microbiota.

Which suggests some of those gut critters are affecting it...not enough being absorbed internally.

I would say berberine acts in the GI Tract and internally, but unknown about specific details, in my mind. Sometimes they discover later what really is happening....

Blind men and an elephant

The parable of the blind men and an elephant is a story of a group of blind men who have never come across an elephant before and who learn and imagine what the elephant is like by touching it. Each blind man feels a different part of the animal's body, but only one part, such as the side or the tusk. They then describe the animal based on their limited experience and their descriptions of the elephant are different from each other.
