That edge was segregated too much when it was poured, the concrete was too wet, travelled too far down the chute, and the fines separated when they vibrated it to fill that edge of the form. I bet they stole a shovel of normal concrete to fill the top without all the holes showing in those cobblestones, or the poor finisher had to pull that much mud off the nearby surface. Best would be to pressure wash it, then use whatever patch, the patch is just keeping the water from pooling and doing more damage. You could do the same thing by chipping drainage channels so it won't hold any water.
