Subject: some questions on the person being treated


I have some questions, and some comments that may help.

Usually the problem is that the insulin is not "working", it is working, but it is really an insulin resistance issue.

I am going to ask if they have the person on metformin ? The metformin will help with the insulin resistance.

There is another medication that can also help with insulin resistance, but atos is out of favor due to cardiac issues.

Second, which insulin(s) are they using ? and how they dose. Is it a sliding dose fallowing meals with a rapid insulin

or is it a "Basel" insulin, such as lantus or tresiba or another ?

It usually takes the body about 2 weeks for the body to reset it self on what the things they can eat or make things better, so if the person

in question can get things under control, you are on your way to success.

or, if needed, cut out the worst thing first, and allow more of the less bad stuff...it took years to get this way, it's going to take a while to get things where they

need to be.

The next stage can get the glp-1 and a few others, which can help modify the the hunger and the like, which also might be an advantage to the person. Sometimes it acts by upsetting

the stomach a little bit, and the person does not eat or eat as much. These meds are now being Rx for weight loss. mounjaro is the "3ed" generation, with another active part added.

It has shown help with blood sugar control, and some liver issues, and is the latest thing on the block.

Is this person consulting with a endocrinologist ? With this kind of indivual, with the issues they have, it is critical to have one on the health team.

There was a question about adhesives, that are a problem. Have both brands of cgm's been tried, the ingredient that is causing the problem may not be in both, something to examine (libre and dexcom).

The last option is rather new, it is the eversense, which is placed under the skin for a year, but there is some sort of receiver or the like, it is supposed to be silicon based. I don't know if that would

cause a problem with the allergy to the glue. It says that most insurance and Medicare will pay.

So, someone just needs to work with the person. Making prepared meals might help, going to town and helping them do their shopping, and help clean out the house. Doing all of this, is not

being like cold turkey....get the doughnuts and the major offenders out of the house first, but still allow them to "live a little", with some things that have sugar, but not near as bad.

Rome was not built in a day, and we need to help them along, one step at a time.
