Brother got talked into it once years ago when he bought a pickup.
Rear end started to make a noise, had a bearing going bad.
They would not cover it because his pickup had a hitch (factory installed) on it, and they said it was a sign of 'abuse' and said he caused the problem by overloading it. Thing is, this was his 'good' pickup, and I don't think he ever had anything bigger than his ATV trailer hooked to it before the trouble - it was within a month or two of him buying it.

Now, on the other hand, we have bought a vehicle from a dealer that includes an extended warranty on every vehicle they sell over $10,000. The warrany was through the dealer, and they will fix what the warranty says they will fix, but you'd better read the fine print on what is not covered, as it is a lot of things. We had a CV boot tear, and when they were putting in the half shaft, they noticed a loose ball joint in the suspension, and also replaced the axle seal (everything was 'dirty' from the torn CV boot and they couldn't tell of the seal was leaking too or not so changed it out of precaution while they were there). The extended warranty paid for the axle seal, and maybe 15 minutes of labor (the additional time to change the seal). The rest wasn't covered because it wasn't in a housing lubricated with oil or fluid.
