It differs from state to state. I Ohio they are salvage, not rebuilt. If you are in a wreck and either parties insurance pays to fix or total your vehicle, you will get less than half the amount than you will with a clean title.

My father in law lived in Arkansas and I bought a motorcycle from him that had a rebuilt title. He explained to me that it was all an insurance and body shop scam. He said if the cops make an accident report, all the vehicles get a rebuilt title. The scam part is in order to get a clean title you have to have a registered body shop do a full restoration. He said nobody does and they don't mind a dent or broken tail light.

This motorcycle I bought was in a wreck and has a few scratches and dings. Nothing you can see from me than 5 feet away. When I got it back to Ohio getting the title in my name was a draw out process with some hoops to jump through. I had to take it to a highway patrol inspection place 20 miles from home. It got a little weird when we got there. The place was a really clean, nice looking shop with lots of tools, a lift, and 2 bays that were big enough for 2 semis with trailers. They told me if they found anything less than perfect in function it would fail. If they found any numbers anywhere that came up as stolen, it would be confiscated on the spot. Then we were escorted to a room with 1 door and no windows and told to sit there and wait. After an hour they came and got us and said everything was OK and we could take it home. When we first got there they were very strict, tight lipped, and stern. After the inspection they were nice and polite. I asked them what they did. They took it out for a ride, checked all the lights, brakes, gauges, and then they took the covers off each side of the engine to get casting numbers and run them through a national database of stolen vehicles. In then end it did get me cheaper insurance.
