Carbohydrates cause the body to hold on to more water than meat protein or fat. When a person goes to a low carb diet the first weight they likely lose is water weight. The first few pounds that come off. This rapid water loss can sometimes cause a person to lose electrolytes like salt and a person can feel kind of bad for a few days because of the electrolyte imbalance. This self corrects after the first week as the body becomes used to the new normal. If a person increases their salt intake during the first week of starting a very low carb diet they can often avoid what is called the "keto flu". It is not really a flu at all but a temporary electrolyed imbalance that kind of has flu like symptoms.
Milk also has carbs from the sugar lactose. So while water will not add to the insulin bolus the pancreas puts out to handle carbs, milk would. The lower the fat content in milk the quicker the lactose gets into blood sugar although the overall glycemic load would not change much. Just the duration of the time it takes to hit the bloodstream. The fat in the milk will slow digestion of the sugar somewhat although not very much.
But yes, the bun and fries would raise insulin and tell the body to store the excess energy as fat. Any energy is excess of the body's immediate needs.
That is my understanding.
One nice thing about McDonalds is it is pretty easy to just order hamburger patties either alone or as additional patties on a regular order and also easy to eliminate the bun on the order of any of their burger selections.
