I'm not a doctor, but it sounds like gout to me.
I would not be so quick to say that New Balance shoes are not cheap junk. In my experience, they used to be good but recently their quality has taken a nosedive. They're still not cheaply priced, but the quality and fit is on a par with cheap shoes. I have switched to Brooks.
