On our farm.... a field showing test results like that would benefit from:

1.  Need to build the soil potassium level to at least double what your soil test shows is currently present.  

2.  If all areas of the field are as low on manganese as your one result shows.... that is concerning.  Last year we started trying to build the  manganese levels to 30 PPM in a few fields.  I can't suggest for you to try the same, because we don't know if the manganese sulfate that was calculated we needed, has had the desired effect yet.  Another year, with follow-up testing should tell us if the applications worked.  But a 5 PPM manganese.... our crops would benefit from incorporating some manganese in our fertility program.

3.  How much, if any drainage tile does this field have?

4.  add crop removal rates for all of the nutrients the crop needs.

5.  A cover crop of clover after wheat harvest does wonders on our farm for soil tilth, soil permeability, and water infiltration.  With the clover either being planted at wheat-seeding time, or frost seeded in the spring.
