A. We just use a tall enough loader. I have in the past when driving pipe taken a pipe with good threads and screwed on a short piece to protect the threads, and driving it down, then screwing the final piece of pipe on to the desired height. Typically that is when we needed something sticking up very high out of the ground, like an overhead brace, or a barn pole.
B. We have some driven fence posts (both wood and steep pipe) that were installed in 2000. As long as the post didn't rot, like the railroad ties did, they are still rock solid.
It is so fast, its a game changer.
