My 1918 house the upper floor is basically an attic/loft are with sloped ceiling/walls.

I gutted it in 2010/11. I took out those filler blocks between the rafters and also the trim on the outside so it was open under the eve. Put in those thin foam chutes from the plate all the way to the ceiling area near the peak. Fiberglass bats went in the inside of the chute between the rafters. Then 3/4 foil faced foam board over that attached to the rafters. Sheet rock over that. On the outside, I cut boards to fit between the rafter tails and put in a grill/screen for air flow. There are roof vents a prior owner installed.

The R value is not awesome I suppose but there just was not room to do much more. It did cut my heating by about 25% and its much more comfortable up there (used to freeze ones tail off and needed an electric blanket). One window AC is sufficient too in the summer.

Originally there would have been no insulation at all and some prior owner had blown in cellulose that was uneven and even had bare spots or voids. Squirrels had been in there and moved some of it around too.
