The odds of a bad rate controller aren't zero, but they are very close to it
The other 99.9% of the time it's improper configuration of settings (especially valve type wrong) or improper sizing of system (too big of nozzle creating too low/high of pressure. I've also seen butterfly valves physically wear out as well
