That hitch allows trailer to rock forward and back over humps and through valleys. No side to side flex like a gooseneck ball allows so flex has to come from semi frame and suspension or the trailer. I would guess trailer would be 1st to break. Air ride truck might allow quite a bit of flex.
It depends on what style of 5th wheel ball is used. The one pictured by the OP, and I’d tend to think, the 5th wheel shouldn’t be pinned/blocked. If you have the other style 5th wheel ball, the ball side of the hitch(being it is of a ball design) allows for various deviations in terrain and truck 5th wheel should be pinned/blocked level.
