How dry have you been since installed? If your rainfall has been low, it may just be subsidence. I had a lot of that happen from tile installed in 2012. No rain, so 2013 & 2014 still kept seeing subsidence.
That said, I would get on the horn and get an inspection done pronto. Installer should be covering their work if that's a true suck hole in this short of time.
And document everything you can in case you and other landowners have to take legal action (from the way you describe this whole project).
