I look at the "as received" columns and ignore the "dry weight" column, as you are going to spread on a wet basis, not dry basis.  Looking down thru the column, I see the moisture is 54.8%.

An on-line calculator says 1 mg/kg = 0.002lbs/ton.  So, take what ever the mg/kg and convert to lbs/ton. For Example, Ammonium N is 65 mg/kg.  That will convert to 0.13 lbs Ammonium N/ ton.

If you want, you can just the calculator and input each number in and make the conversion.
