We did this. Dig down below frost and formed.
Also helped a neighbor form one in dirt and only grade formed the top with a 2x6.

We used a 2x2 angle iron. Welded rebar sticking straight out from the inside corner of angle iron into the form area. Suspended from cross blocks across the form with bolts that were cut off when ready to remove forms.

Basically grade must be set so the lower step of the form is just cleared by the bottom of the doors. They would then run against this angle iron on the back side

Iron is just to protect the edge of the 2" lip step and keep it from breaking over time.

Form would look like this, flat 2x6 for under doors. Angle iron (shown as parentheses) on top front corner.
