often receive no coins. the seasoned wait staff will give your change back in a variety of denominations.
more often than not, I put the bill amount plus tip in the little book, then tell them no change please.

I would say most people use a card and just check a little box in the tip column.

Mostly just old schoolers use cash anymore ....... Me included :)
