I don't do X (Twitter) so have not heard it but I did a search on Youtube and "Doctors to Trust" series has a bunch of shorter clips from his previous interviews or presentations concerning Alzheimer's being a metabolic disease. Here is what a search comes up with:

There has been quite a bit of evidence over several years now that ketones can fuel the brain and improve cognition in sufferers of Alzheimer's. So it appears that it is a glucose fueling problem with the brain at least to some extent. Diabetes and insulin resistance appears to be a big risk factor in its development.
