Anything that lets in air could be the cause; a worn shaft, a poor fitting gasket, a poorly adjusted air screw...
Then there's ignition to test and then fuel delivery. If you can cover them all, it will no longer be a mystery.
P.S. Also do a compression test if all else fails. That's another possible cause.
