As I left the trades extension cord safety became a big thing, I had a safety officer at Argon national labs spend 2 hrs going over equipment we were bringing in for a 2 week job for 2 guys.
He went over the cords inch by inch visually and then used a plug in cord tester.
Extension cords on union sites now are inspected monthly and tagged if they pass.
I still use electrical tape on outer jacket repair and will replace ends when they get loose or lose the ground prong
